Joined Up Care Derbyshire – Five Year Strategic Plan

The members of the Joined Up Care Derbyshire Board (Derby and Derbyshire’s Sustainability and Transformation Partnership STP),would like to invite you to an event to discuss the ‘draft’ refreshed STP five-year strategic plan.

Venue: The Whitworth Centre, Ballroom, Station Road, Darley Dale, Matlock DE4 2EQ. Onsight free parking is available. Please use the car park to the side of the building.

The refreshed plan will demonstrate Derbyshire’s commitment to the ambitions set out in the national NHS Long Term Plan and outlines how these will be implemented locally. Its focus will be on:

  • Ensuring the citizens of Derbyshire ‘have the best start in life, stay well, age well and die well’.
  • Addressing inequalities and population health management.
  • Working together to make improvement in the wider determinants of health such as housing, education and air pollution management leading to improved outcomes for people in Derbyshire.
  • Addressing inequalities and population health management.
  • Working with people not just patients, i.e. a focus on wellbeing not just ill health.

In addition to finding out about the ambitions of the NHS Long Term plan there will be the opportunity to hear about and discuss why Joined Up Care Derbyshire needs to change the way services are delivered, the model of care, how the wider determinants of health will be considered, the move to an Integrated Care System (ICS), the implementation of digital advances, the plans to develop the workforce, the development of Place Alliances and Primary Care Networks, and the financial challenge.

This event builds on extensive engagement that has already taken place to inform the draft submissions to the plan, throughout the work-streams, the Place Alliances, through the JUCD Citizens Panel (August and September), out and about in the local community via the interactive workshop, through the work with Creative Carers to establish ‘what carers want’, and through the work of Healthwatch Derby and Derbyshire. It is part of an ongoing commitment to involving and communicating with the public, communities, and staff over the next 5 years, to demonstrate progress.

Draft plans will be submitted to NHS England and NHS Improvement on the 27th September 2019, and full and finalised plans need to be agreed by the 15th November 2019, so this is your opportunity to be part of an open, and transparent dialogue about the plans content and implementation, and raise any areas of concern, or ideas for improvement, to ensure what matters most to you, has been captured and shared.
