Message from Joined Up Care Derbyshire

Joined Up Care Derbyshire (JUCD) brings together health and social care organisations across Derbyshire. Working together more closely than ever before, the ambition is to provide the best care and services for people and make them as efficient and effective as possible. As a direct result of JUCD, Place Alliances were formed; commissioners, community services providers, local authorities, primary care, the voluntary and community sector, and the public working together to meet the needs of local people.

Derbyshire Dales Place Alliance and Derbyshire Dales Health and Wellbeing Partnership have been successfully working closely together over the last couple of years empowering people to live a healthy life for as long as possible through joining up health, care and community support for citizens and individual communities. Whilst understandably refocused on to the COVID -19 response, now, more than ever, the joined up approach in Derbyshire Dales is essential and partners are working closely together regardless of organisational boundaries.

One partner, Derbyshire Dales CVS, in association with their members, in particular Connex, is providing coordination of community voluntary services. These services have always been and continue to be essential in providing support across the system. Social Prescribers are working closely with General Practices in Derbyshire Dales, signposting and providing information and support to people and organisations.

As an Alliance, we meet (virtually) twice weekly to discuss community resilience – identifying potential gaps in provision, acting as a support network for partners and sharing solutions. We very much appreciate Derbyshire Dales CVS kind offer to the group to have a regular space in this publication. We will keep you updated and share useful information

Request for help regarding emergency accommodation;
“Councils across Derbyshire County are appealing to property owners that might have habitable property available for individuals or households that find themselves with nowhere to live. Any landlords or property owners should contact DASH Services that are coordinating any responses. They are available on 01332 641408 or email Or alternatively make contact with your local District/Borough Council. They will need access to accommodation not just now but over coming weeks. With hotels closed, options are rapidly diminishing.”
